    Helios 522 was a very interesting crash. The mechanic did some maintenance on the oxygen system, which required shutting it down, and then forgot to turn the oxygen back on. Therefore, when the plane next flew, it did not generate oxygen, and by the time the ground crews had figured out the problem, the pilots were incapacitated...
