For airliners, its when a positive rate of climb has been verified on the vertical speed indicator or whatever its called.
Absolutely. Perhaps one could use a high speed train with a third rail, getting to around 200km/h before launching the plane from a catapult.

Well from my point of view, control surfaces work better. And besides, rotators are usually quite floppy.
