this happens when the speed goes crazy, often if the speed will go over about 5000 - 7500mph then this happens, I think its just the game

Set the front wings waaaay back. Make sure they're semi symmetric. And instead of having a shit Ton of fuel blocks, try using fuselage blocks and use those for fuel.
