The Crash of Flight 6237:

It's a lightly foggy evening as Wright International Airways Flight 6237 Departs from Yeager Int. airport On route to Wright Int. Airport for it's weekly cargo service. The crew is heard on the cockpit voice recorder talking about their personal lives throughout the fifteen minute cruise portion of the flight. The Captain is the pilot flying the plane and the First Officer is the pilot monitoring the instruments. The First Officer Requests the downwind approach for runway 36 from Wright Approach. This type of approach into Wright calls for pilots to fly the plane to runway 18, then take a left turn onto a 220 degree heading, then turning back onto heading 180, essentially putting the plane on the left parallel with the runway. After flying 15 miles parallel, the crew take a 90 degree right turn onto heading 270, then shortly after, taking another 90 degree right turn to line up with runway 36, all the while descending to 2,000ft. Flight 6237 is cleared for the Instrument Landing System (ILS) runway 36 downwind approach, then told to contact the Wright Airport Tower frequency. The crew makes the final turn onto the approach and the Captain turns on the ILS system, at which point He says to the First Officer "Weather's gotten worse, you can't see a thing in this fog. We won't catch sight of the runway until we're right on top of it." The pilots have to trust the on-board ILS system in order to guide the plane into the touchdown zone. litle do they know, the Captain has made a grave error. "Final Approach Checklist complete!" says the First Officer, they are about 3 miles out from landing. "OK, 4 miles left to go." says the First Officer. Neither of the pilots have realized they've intercepted the ILS system for runway 18 instead of runway 36. This means the pilots are going to land on the wrong side on the runway and quickly overshoot. "500" The GPWS starts it's call-outs as the plane descends.

"What!?" The Captain shouts just as he realizes his mistake.
"GO AROUND!!"
"Oh - - - - !" The pilots shout as they desperately try to recover their aircraft, but to no avail. The plane Shoots off the end of the runway and crashes into the ocean below. Both of the pilots die on impact as their plane tears itself apart. It's quickly apparent that the Captain intercepted the wrong ILS approach, so the FAA ordered that the ILS system buttons be spread out across the console rather then being exclusively on the Captain's side.
