In most nuclear-powered rocket engine designs, any radioactive material at all being in the exhaust is a very bad thing, because they're generally designed such that this doesn't happen so if it is happening you're probably experiencing a nuclear meltdown. There are designs where radioactive material in the exhaust would be intentional, such as the open cycle gas-core NTR -- but as Atomic Rockets very eloquently puts it, that "more or less makes the exhaust plume a weapon of mass destruction".
