The CSX 8888 Incident, also known as the Crazy Eights Incident, was a freight train accident in Ohio on May 15, 2001.

The incident occurred after an operator got off the locomotive, which was still at low speed, to change tracks and maneuver another locomotive in the yard. The problem was that he didn't apply the brake
